Yes Paul; you are on the button with all three points. Unfortnately our host's (Pliner's) system cannot be switched to prompt for a unique pseudonym.
As you know there are a wide parameter of users on the SAEF, from raw beginner to advanced initiates, but often an experienced occultist can be a newbie at the net and make mistakes until they get used to the system. I have even had people complain that they have posted a message to the thread and that I have deleted it when I have done no such thing. What has happened is that the newbie has clicked on the email address of the person making the original forum message and unwittingly sent an email to them! (this is a facility which can be helpful to users for you can email individuals privately without having your message reflected in the thread). Similarly, the unthinking use of 'anonymous' can unintentionally soon get people at cross purposes. Some forum users have asked if it would be better if the forum were set to force people to put in their genuine email addresses but we feel that would mitigate against newbies who may feel intimidated but who are ready to participate if they feel their comments cannot be attributed. The upshot is that naive users can get confused about forum facilities and this coupled with such sensitive areas of discussion sometimes leads them to feel excluded or prejudiced against - hence their occasional outbursts. Naturally we always make clear that our FIRST AND FOREMOST intention is to be as empathic as possible to everyone's opinions and find a solution or answer for them so beginners should keep in mind that if it looks like we are being elitist it may be something they themselves have caused and a requesting clarification would be a wise next step. This issue seems to be about a couple of things:

1) Awareness - a new person posting may not be aware of this request
2) Logic - so we know who is saying what.
3) Responsibility - seperates the Adepts from the neophytes.

Denise, is there no background script setting that ensures users put in a user name (excluding anonymous?).

This issue seems to be cropping up again and again, hence I figure a post, might be of use, but delete if not. :)
